It depends whether it has a substantial bezel or not. Diver - 40mm. All dial dress watch - 38mm.
Depends on wrist, and lug to lug is equally important.
The size that has no over hang with the lugs works for me. I like all shapes and sizes after that requirement is met.

No such thing. I wear 37-43 comfortably and I wouldn't change the size of any of my watches.
Depends on the watch type
Dress watches I usually go 38 and dive and pilot watch 42 - 45 on my 7 1/4 wrist. Most others 40-42 for me.
